Forms should be printed off, filled in and taken to the Embassy.
Application forms are available free of charge from the Embassy. You may use the electronic version available on this website, or the Embassy can post a copy to you, provided you send a pre-stamped and self-addressed envelope to:
- The Consular Office, Embassy of the Federal Democratic Republic of Ethiopia, 17 Princes Gate, London SW7 1PZ.
- We cannot accept completed forms submitted by email.
- If you are experiencing problems accessing the online form, please contact the Embassy on info@ethioembassy.org.uk and we will send you a copy of the form by email.
Completing the New or Replacement Passport Application Form
First-time Ethiopian Passports
To apply for a new (and first) Ethiopian passport, please read the following information carefully.
1. Answer All of the questions on the form.
- Answers must be either typed in or printed in the spaces provided on the form, using BLACK or BLUE ink only.
- Failure to answer ALL questions will result in the Embassy being unable to process your application or issue a passport.
- TWO COPIES of the application must be submitted.
2. Submit the fee of £36. For students the fee is £18*.
*students applying for a new passport must submit a letter from their respective university/college, and must bring an ID bearing their photograph.
The following methods of payment are accepted:
- Cash
- Banker's Drafts
- Postal Orders - made payable to The Ethiopian Embassy.
- Payment by cheque or any means other than those stated above, cannot be accepted.
3. Attach FOUR recent passport-sized colour photographs, using a paper clip.
- Do not forget to write your name on the reverse of the photographs.
4. Applicants must produce documents that certify their citizenship. (Documents allowed can be your previous passport, ID card, Birth Certificate, etc.)
5. Submit your application form, photographs and fee.
All applications must be made in person as applicants will be required to give fingerprints at the Embassy
Applications must go to:

- Provided you have submitted all the required documentation and the correct fee, the Consular Office can normally process your application within three working days from receipt of your completed form.

New passport for Children born abroad
1. The Parent should answer All of the questions on the form.
- Answers must be either typed in or printed in the spaces provided on the form, using BLACK or BLUE ink only.
- Failure to answer ALL questions will result in the Embassy being unable to process your application or issue a passport.
- TWO COPIES of the application must be submitted.
2. Submit the fee of £36.

4. The child's parents must produce documents that certify their citizenship. (Documents allowed can be passport, ID card, Birth Certificate, etc.) COPIES OF THE DOCUMENT MUST ALSO BE SUBMITTED.
5. A copy of the Child's Birth Certificate will be required.
6. Submit your application form, photographs and fee.
All applications must be made in person.
Applications must go to:

Replacement of lost or stolen Ethiopian passports
To apply for the replacement of a lost or stolen Ethiopian passport, please read the following information carefully.
1. Answer ALL questions on the application form.
- Answers must be either typed or printed in the spaces provided on the form, using BLACK or BLUE ink only.
- Failure to answer ALL questions will result in the Embassy being unable to process your application or issue a passport.
- TWO COPIES of the application must be submitted.
- The applicant must write a letter to the Embassy stating that he/she is not a citizen of another country
- Applicants must produce documents that certify their citizenship. (Documents allowed are ID Cards, Birth Certificates, Driving Licence, etc.)
*students applying for a new passport must submit a letter from their respective university/college, and must bring an ID bearing their photograph.
The following methods of payment are accepted:
- Cash
- Banker's Drafts
- Postal Orders - made payable to The Ethiopian Embassy.
- Payment by cheque or any means other than those stated above, cannot be accepted.
3. Attach two recent passport-sized colour photographs, using a paper clip.
- Do not forget to write your name on the reverse of both photographs.
4. Submit your application form, photographs and fee.
5. A copy of the police report detailing the circumstances of the loss of your passport, must also be submitted.
Applications must go to:

- Provided you have submitted all the required documentation and the correct fee, the Consular Office can normally process your application within three working days from receipt of your completed form.
